Music Rights Licensee Report

This form is used by Churches to report their usage of Music Rights songs.

Instructions:

This form does not auto save. You must hit “submit” to save your entry
Please use a separate form for every QUARTER you are reporting. You can, however, report weekly or monthly if you prefer.
If you are unsure of the songwriter, just put the song title

Note: if you mistakenly add a performance at the end and then delete it, the form might not submit.
The remedy is to click Add Performance again and simply enter some bogus values in all the fields, such as “Ignore” and then submit.

Tips and Notes

After you submit the form, you will receive a confirmation email summarizing what you just submitted.

This form can’t be edited once it is submitted. You need to submit the form after each session. If you don’t Submit the form, your input will not be saved.

We suggest filling out this form once per quarter, at the end of the quarter including all of the songs used in that quarter.

If you would prefer to submit on a more frequent basis, that’s no problem. Add as many performances as you like and submit when you are done with that session. If you don’t Submit, your input will not be saved.
